RECLAIMsleep is an annual care membership program designed to take the stress out of managing CPAP therapy. Our goal is to ensure every patient feels supported, properly supplied, and never alone throughout their sleep therapy journey.

Through this membership, patients receive ongoing care, guidance, and access to the services included in their selected RECLAIMsleep package.

Important:
Before completing this form, please ensure that:

1. The patient has been fully educated about the selected membership package and understands the benefits included.
2. Membership payment is completed upon selecting RECLAIMsleep+ Premium.
3. A Member ID has been generated and is ready to be registered in our system

Once the form is submitted with the correct details, the patient will be officially registered under the RECLAIMsleep Membership Program.
